Anticoagulation in Atrial Fibrillation with Prior Intracranial Haemorrhage
<p>This research article presents a <strong>systematic review and meta-analysis</strong> evaluating the use of <strong>direct oral anticoagulants (DOACs)</strong> versus <strong>vitamin K antagonists (VKAs)</strong> in patients with <strong>atrial fibrillation</strong> who have previously suffered an <strong>intracranial hemorrhage</strong>. Analyzing data from five large observational studies, the authors found that <strong>DOACs</strong> are associated with a <strong>reduced risk of recurrent bleeding</strong>, <strong>ischaemic stroke</strong>, and <strong>all-cause mortality</strong> compared to traditional blood thinners. However, the study notes that the <strong>certainty of evidence</strong> remains moderate to very low due to the potential for <strong>residual confounding</strong> and the lack of randomized controlled trials for this specific high-risk group. The findings suggest that while <strong>DOACs</strong> appear to be a safer and more effective alternative, treatment decisions should be <strong>individualized</strong> rather than based on a definitive change in clinical practice. Ultimately, the researchers call for more <strong>prospective evidence</strong> and <strong>stratified analyses</strong> to better guide clinicians balancing the complex risks of stroke prevention and recurrent brain bleeds.</p>
