JAMA Cardiology October 2026 Intensive vs Standard Blood Pressure Lowering in People With and Without Chronic Kidney Disease
recent systematic review and meta-analysis investigated how aggressively reducing blood pressure impacts heart health and renal function in adults. By examining twenty-seven randomized trials encompassing nearly ninety thousand participants, researchers compared intensive and standard blood pressure management strategies. The findings revealed that aggressive blood pressure reduction significantly lowers cardiovascular event risks by fifteen percent for every ten-millimeter decrease in systolic pressure, regardless of whether a patient has underlying kidney disease. Although this aggressive approach raised the likelihood of moderate, sustained declines in estimated glomerular filtration rates, it did not increase the actual incidence of kidney failure. Because baseline heart disease risks are elevated among kidney patients, the absolute cardiovascular benefits are especially pronounced in this group. Ultimately, the study concludes that intensive blood pressure lowering should be broadly recommended for individuals both with and without chronic kidney disease.
